Fine to call here?

I struggle to work these things out in 15 seconds, but if he has one pair I'm a slight favourite, if he has two pair I'm a slight dog, and if he has a set I'm 2-1.

My question is should I ever consider how many of my outs have already been folded before I make a call?

I snap call, I think it's really unlikely you're gonna walk into a set here, you've definitely got 4 Kings, and 8 hearts (I discounted the Kh) and you will more often than not count the 3 Aces and the 3 Queens.

What I'm thinking is 5 hands have already folded.  What are the odds of some hearts or Kings having already been folded?  I can't think of how to work this out.  Surely this should impact on my decision, although I don't think I have ever heard it being discussed before.

2-1 dog vs. a set isn't peanuts, but I take your point.

You can't ever calculate folded cards in your odds because you just don't know. You could equally say, what if everyone folded 72, 56, 49 etc so that increases my odds of hitting a K, you just can't do it.

So you work out your outs purely based on what you KNOW.... the community cards and your cards.

So you definitely have 12 outs, and probably alot more likely 18 outs which is not far off half the remaining pack lol.
