    "get it in bad" means you are not the favourite when you put your money in.

    Circumstances (relative stack sizes, pot committed & many others) mean we must get it in bad from time to time.

    And that includes 40/60's. Sometimes the maths say we MUST get it in with a 40/60.

    Finally, if you ALWAYS get it in good, or ONLY call when you are ahead, then you are doing it wrong.
